Authorization To Release Remains

This document needs to be completed, signed, and faxed to Miller-Dutra Coastside Chapel before we can transport the decedent to our facility.

Arrangement Preferences

This document is useful for preplanning funeral arrangments. Bring the completed form with you when you meet your Miller-Dutra funeral director, or you can fax, email, or mail the document to your funeral director. All information will be held in strict confidence.
